16: ABI and Analog Pedal Artery Waveforms

Introduction

This chapter introduces ABI and Analog Pedal Artery Waveforms (ABI) — a simple, noninvasive test that compares the systolic pressure at the level of the ankle to the systolic pressure at the level of the brachial artery. Continuous-wave (CW) analog Doppler waveforms are recorded to support the pressure information.

Student Learning Objectives

By the end of this lesson, students will be able to:

  • Explain the purpose and principle of lower-extremity ABI and analog pedal artery waveform testing.
  • Identify appropriate indications and contraindications for the exam.
  • Demonstrate proper cuff placement, Doppler probe selection, and technique.
  • Interpret waveform patterns and pressure gradients.
  • Recognize disease severity and differentiate normal from abnormal findings.
  • Correlate physiologic results with clinical presentation and duplex findings.
